Endolift® works by placing a very fine optical laser fibre just beneath the skin and delivering gentle, controlled heat directly into the tissue that holds the lower face up. That heat does two things at once: it tightens and retracts the connective septae, the supportive strands under the skin, and it stimulates the skin's own collagen-building response. Small, stubborn pockets of fat in the same area can be reduced at the same time. The immediate retraction is modest. The result people actually notice builds over roughly six months as new collagen matures.
This guide explains what is happening under the skin, what the treatment feels like, how results appear over time, and, just as importantly, the point at which Endolift® stops being the right answer. What does the Endolift laser actually do under the skin?
Endolift® is a minimally invasive laser treatment, not a treatment applied to the surface of the skin. Under local anaesthetic, a fibre finer than a hair is introduced into the sub-dermal layer and moved through the treatment area, delivering heat where the supporting tissue sits rather than through the skin from above.
Three separate effects follow, and it helps to keep them apart because they appear on different timescales.

This is the distinction that matters when you compare Endolift® with surface-energy devices. Ultrasound and radiofrequency treatments deliver energy through the skin towards a target depth. Endolift® starts at the depth. That is why it is described as minimally invasive rather than non-invasive, why it needs local anaesthetic, and why it tends to produce a firmer result on early-to-moderate laxity than needle-free options.
The fair measure of Endolift® is not how tight your face looks the next morning. It is how the lower third of your face sits six months later, when the collagen you have built has finished maturing.
Which areas can Endolift treat, and which are off the list?
Endolift® at Facial Sculpting is used on the mid to lower face, the mandibular border (the jawline), and under the chin. The neck can be included in a combined face and neck plan. Those areas share a common feature: skin that has begun to lose its grip on the structure beneath it, without so much excess skin that only surgery will resolve it.
- Lower face and jawline. The core indication. Softening definition along the mandibular border and early jowling respond best.
- Under the chin. Useful where there is discrete submental fullness with some skin laxity over it, rather than a heavy double chin.
- Neck. Early to moderate laxity of the upper neck, treated as part of the face and neck plan.
- The lower eyelid. Not treated with Endolift® here. Dr Nina refers lower-eyelid concerns to an oculoplastic surgeon with specialist training in that area. This is a deliberate safety position rather than a limitation of the device.
If your concern is skin quality rather than structure, a different route usually serves you better. Loss of glow, crepiness and dehydration respond to Profhilo® or polynucleotides, and those are frequently sequenced alongside Endolift® rather than instead of it. If the concern is proportion rather than laxity, that is face shape sculpting territory.
What happens on the day, and does it hurt?
The area is mapped, then numbed with local anaesthetic, so the treatment itself is not painful. Most patients describe pressure and movement rather than pain. There are no scalpel incisions and no general anaesthetic, and you go home the same day.
- Consultation first. Endolift® is never booked on the day. A £200 consultation covers your medical history, a facial-anatomy assessment, whether Endolift® is genuinely the right option, realistic outcomes, alternatives, consent, and your exact plan and price.
- Mapping and anaesthetic. The treatment areas are marked and local anaesthetic is given.
- The fibre is placed. The optical fibre is introduced just beneath the skin and worked through the planned area, delivering controlled heat.
- Immediate retraction. The connective tissue tightens as it is treated. Any early lift you see is partly this, and partly swelling.
- Aftercare and review. You leave with tailored aftercare advice, and a review can be arranged so your progress is tracked as collagen builds.
How long is the downtime after Endolift?
Downtime is minimal, and most patients return to normal activities within about three days. Temporary swelling, bruising, redness, tenderness or numbness at the treated area are the usual short-term effects and generally settle quickly. Less common risks are discussed individually at consultation, because they depend on your anatomy and medical history.
Plan around the three days rather than around the treatment itself. If you have an event you would not want to attend with visible swelling along the jaw, leave two clear weeks between treatment and the date, and say so at consultation so the timing can be built into your plan.
When do Endolift results actually show?
Results build rather than arrive. The expectation timeline we set out at consultation runs like this.

Endolift® is long-lasting but not permanent. Because the result rests on collagen you have built yourself, how long it holds depends on your anatomy, your skin quality, sun exposure and general skincare. Individual results vary, and nobody can promise a number of years in advance.
Who is Endolift right for, and who should be told no?
The treatment suits adults with early to moderate laxity of the lower face who want firmer definition without surgery, and who are comfortable with a gradual, natural change rather than a dramatic one. It is a poor choice for anyone hoping for a surgical-level lift.

Endolift® may also be unsuitable during pregnancy or breastfeeding, where there is active infection or a skin condition in the treatment area, or alongside certain medical conditions identified at consultation. Dr Nina's stated position is that she will tell you when the treatment is not the best option for you, including when the right recommendation is a surgical opinion instead.
How does Endolift compare with HIFU, EMFACE and a surgical facelift?
The useful comparison is invasiveness against what each can realistically shift. There is no single best device, only the right one for the degree of laxity in front of us.

In practice these are combined more often than they are chosen against each other. Endolift® is frequently planned alongside a skin-quality treatment such as Profhilo® or polynucleotides, or with EMFACE® for tone, in a sequence agreed at consultation. What Endolift cannot do
Stating the limits plainly is part of the assessment, not a caveat bolted on at the end.
- It cannot remove significant excess skin, and it does not replace a surgical facelift.
- It cannot correct advanced sagging or marked jowling on its own.
- It is not used on the lower eyelid, which is referred to an oculoplastic surgeon.
- It does not deliver instant, dramatic change. Results build over months.
How much does Endolift cost in London?
At Facial Sculpting, Endolift® starts from £3,900 for the face plan and from £4,900 for the face and neck plan. An Endolift® consultation is £200 and is required before treatment. Is Endolift painful?
The treatment is performed under local anaesthetic, so the area is numbed throughout. Most patients tolerate it well and describe pressure rather than pain. Tenderness afterwards is usually mild and short-lived.
Is one session of Endolift enough?
For most patients treated for early to moderate laxity, a single treatment is planned and then reviewed as collagen builds. Whether anything further is useful is a decision taken at review, once the six-month result is visible, rather than assumed in advance.
Is Endolift suitable for jowls?
For early jowls and a softening jawline without significant skin excess, yes. Where jowling is more advanced or accompanied by loose skin, Endolift® on its own will underdeliver and a surgical opinion is the more useful route.
How long does Endolift last?
Results are long-lasting rather than permanent. Because they rest on your own collagen, they can hold for years with good skincare and occasional maintenance, though the duration varies with anatomy, lifestyle and skin quality.
Can Endolift be combined with other treatments?
Yes, and it frequently is. Skin boosters such as Profhilo® or polynucleotides, energy devices such as EMFACE® or InMode Contoura, and injectable contouring are all commonly planned around it. What is safe to combine, and in what order, is agreed at consultation.
